adults (0.5 mL) in single-dose pre-filled syringes.

- To inform healthcare providers about the reinstatement of universal purchase
of all childhood vaccines in Washington.

- December 18 - This health advisory (PDF) has two purposes:
- To inform physicians and other healthcare providers how to return recalled
lots of Sanofi Pasteur H1N1 Pediatric Vaccine to the manufacturer.
- To encourage providers to help prevent future pandemic waves by vaccinating
all persons older than 6 months of age now that a substantial supply of
vaccine is available.
- December 15 - Non-safety
related voluntary recall of certain lots of Sanofi Pasteur H1N1 Pediatric (0.25 mL,

How to order H1N1 vaccine
H1N1 vaccine can be ordered from your local health jurisdiction. Submit a vaccine request by e-mail or FAX as indicated on the forms below. Orders will be filled depending on vaccine availability. If you have questions, please call (360) 852-7691 or e-mail ph.message@clark.wa.gov.
Reporting forms
To ensure vaccine is provided to the community as efficiently and effectively as possible, providers in Clark, Cowlitz, Skamania, and Wahkiakum counties are required to report to Region IV Public Health vaccine inventory, number of doses administered, vaccine transferred to other providers, and other information.
